Grades and Transcripts
To request an official transcript, please follow the steps below.
- Complete a Transcript Request Form: The form is available at the front desk.
- Pay the Fee: A $20.00 fee is required for each transcript request.
- State Your Reason: Indicate the reason for your request on the form.
- Ensure Good Financial Standing: Official transcripts will only be released to students who are in good financial standing with the school.
- Other Service Requests: Any additional service requests will incur a $50.00 fee.
Please note that Ultimate Health School reserves the right to withhold official transcripts from third parties. Processing time for official transcripts is typically 7 business days.
