Age Requirement

Applicants must be at least 18 years old or turn 18 before their first clinical in Fundamentals of Nursing. Valid proof of age is required (e.g., passport, driver’s license, state ID).
Social Security Card
A copy of your Social Security card is needed. A tax ID number may be temporarily accepted while you are working on obtaining a Social Security number.
Attestations
If you do not provide a Social Security card, you must sign attestations regarding disclosures.
Licensing Board Verification
Applicants must verify acceptable forms of identification with their state licensing board before taking the NCLEX examination.
Drug and Background Checks
A mandatory urine drug test and criminal background check are required for all applicants.
CPR Certification
A valid CPR BLS (Basic Life Support) card from the American Heart Association is required.
Additional Requirements for Practical Nursing Applicants
Letters of Recommendation
Two (2) letters of recommendation are required.
Official High School Transcript or GED
An official high school transcript or GED is necessary. If the transcript is in a language other than English, it must be translated and evaluated for U.S. equivalency. Applicants may also provide proof of post-secondary enrollment as evidence of high school completion. You have up to 45 days after the start of classes to submit the official documents.
High School Completion Waiver
Applicants from countries experiencing crises or conflict may be exempt from the high school completion requirement if they score at least 70% on the admission exam. English proficiency will also be evaluated through the exam.
Health Requirements (Prior to Clinical Experience)
- TB Test (PPD or Chest X-ray): Must be conducted within the last year. (Yearly)
- Hepatitis B: Provide proof of titers or declination.
- MMR (Measles, Mumps, Rubella)
- Varicella (Chickenpox)
- TDAP (Tetanus, Diphtheria, Pertussis)
- Flu Vaccine: Required or a declination note must be submitted. (Yearly)
- COVID-19 Vaccine: Strongly encouraged.
Criminal Conviction Disclosure
Certain criminal convictions may affect eligibility for licensure in Virginia. Applicants are encouraged to consult with the Virginia Board of Nursing if they have concerns. Practical Nursing applicants are required to sign a criminal conviction notice.
Conditional Admission Policy
UHS offers conditional admission for applicants who score between 65-69 on the HESI Entrance Exam. Up to ten (10) students per cohort may be admitted under this policy. Those admitted conditionally must complete a remediation program that includes:
- Enrollment in structured academic support and tutoring.
- Participation in supplemental learning activities.
- Successful completion of designated assessments during Term 1.
Failure to meet remediation benchmarks may result in dismissal from the program. This policy aligns with the standards set by the Accrediting Bureau of Health Education Schools (ABHES).
