Grades and Transcripts

To request an official transcript, please follow these steps: 

1. Complete a Transcript Request Form: Available at the front desk. 
2. Pay the Fee: A $20.00 fee is required for each transcript request. 
3. State Your Reason: Indicate the reason for your request on the form. 
4. Ensure Good Financial Standing: Official transcripts will only be released to students who are in good financial standing with the school. 
5. Other Service Requests: Any additional service requests will incur a $50.00 fee. 

Please note, Ultimate Health School reserves the right to withhold official transcripts from third parties. Processing time for official transcripts is typically 7 business days.
